Account Security

You are responsible for:

• Maintaining the confidentiality of your account credentials
• All activities that occur under your account
• Notifying us immediately of any unauthorized access
• Ensuring your account information is accurate and up-to-date

Prohibited Uses

You agree not to:

• Share your account with others
• Order tests on behalf of another person without proper authorization
• Provide false or misleading information
• Attempt to circumvent security measures
• Use the Service for any illegal purpose
• Interfere with or disrupt the Service

5. Lab Testing Services

How Our Service Works

1. You select and purchase lab tests through our platform
2. A licensed physician reviews and approves your order
3. You visit a Valley Diagnostics lab location for sample collection
4. Samples are processed by our certified laboratory partners
5. Results are delivered to you electronically through our platform

Physician Review

All orders require physician approval. The reviewing physician:

• Is an independent, licensed physician
• Reviews orders to ensure medical appropriateness
• May decline orders that are not medically appropriate
• Does not establish an ongoing doctor-patient relationship

Test Limitations

Laboratory tests have inherent limitations:

• Accuracy: No test is 100% accurate. False positives and false negatives can occur.
• Timing: Test results reflect your status at the time of sample collection and may change over time.
• Interpretation: Results must be interpreted in clinical context by a qualified healthcare provider.
• Not Comprehensive: Tests only measure specific markers and do not provide a complete health assessment.

Sample Collection Requirements

You must:

• Visit a Valley Diagnostics location for sample collection
• Bring valid photo identification
• Follow any pre-test instructions (fasting, medication restrictions, etc.)
• Complete sample collection within the timeframe specified in your order

6. Pricing and Payment

Pricing

• All prices are in U.S. Dollars and are subject to change
• Prices displayed include all fees; there are no hidden charges
• Special offers and discounts may have specific terms and conditions

Payment

• Payment is required at the time of order
• We accept major credit cards and select payment methods as displayed at checkout
• You authorize us to charge your payment method for all fees
• Failed payments may result in order cancellation

No Insurance Billing

Labs4me is a direct-to-consumer service. We do not bill insurance companies. All payments are your responsibility. Some tests may be eligible for reimbursement through your health insurance or HSA/FSA; you should check with your provider.

7. Cancellations and Refunds

Before Sample Collection

You may cancel your order and receive a full refund if:

• You cancel within 24 hours of placing the order
• The physician declines to approve your order
• We are unable to fulfill your order for any reason

After Sample Collection

Once your sample has been collected, orders cannot be cancelled and no refunds will be issued. This is because:

• Laboratory processing costs have been incurred
• Samples cannot be returned or reused
• Test results cannot be "ungenerated"

Refund Processing

Approved refunds will be processed within 5-10 business days to your original payment method.

8. Results Delivery

Turnaround Time

• Most results are available within 1-3 business days after sample collection
• Some specialized tests may take longer
• Turnaround times are estimates and not guarantees

Access to Results

• Results are delivered electronically through your account portal
• You will receive an email notification when results are available
• Results remain accessible in your account indefinitely
• You may download or print your results at any time

Abnormal Results

If your results contain abnormal findings, we strongly recommend consulting with a healthcare provider immediately for proper evaluation and follow-up care.

HIPAA Authorization

Governing Law

These Terms are governed by the laws of the State of California, without regard to conflict of law principles.

Arbitration

Any disputes arising from these Terms or your use of the Service shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association, except where prohibited by law.

Class Action Waiver

You agree to resolve disputes on an individual basis only. You waive any right to participate in class action lawsuits or class-wide arbitration.
